Faculty Schedule Requests

As of September 1st, 2003, all scheduling requests may submitted through our internet scheduling site at www.net-practice.com . Submitting requests in this way will allow you to track your requests through the scheduling process. Requests may also be submitted by e-mail to Anesthesiology.Scheduler@hsc.utah.edu.

To make a request on the scheduling web site, after logging on to the system, select "Scheduling", then " Make a Schedule Request", then fill out the form. Under comments, please elaborate on any extenuating circumstances which might exist regarding your request, and include the date you are submitting your request. Please keep in mind your request and comments may be viewed by other personnel in the department.

Your request will automatically be placed in queue according to order of submission and marked as "Pending" under the "Request Queue" section of the web site. Once the scheduler has had time to review these requests, they will be marked as either "Accepted" or "Denied." If the request is accepted, it is entered into the scheduling database and is then viewable under "Request History." At this point, it is still a REQUEST and not a guaranteed assignment unless you have categorized it as vacation. Many requests cannot be given a final disposition until the actual schedule is created. Requests submitted by e-mail, once entered into the scheduling system, will be viewable under "Request History."

Once vacation time is placed in the "Request History" section of the web site by the scheduler, it is guaranteed time off. You may make firm plans, buy plane tickets, etc, at this point if your time is categorized as VACATION. All other requests are not firm or guaranteed until the final schedule is created.
